Onboarding for Librarians and Staff

Confirm Your Sign-in Appointment  

Once you've accepted a job with us, Library Human Resources (LHR) will email you to confirm your start date and schedule your sign-in appointment.

Complete Employee Information Input Form

Complete the Employee Information Input Form and submit to LHR by the assigned deadline. Reflect your name as it appears on your social security card on the form. LHR will then generate UCLA and UCPATH employee ID numbers for you.

Create Your UCLA Logon ID

After you've completed Step 2, LHR will email you your UCLA employee ID number which you will need in order to create your UCLA Logon ID. Standard format for a username is the initial of your first name followed by your full last name (ex: John Smith's username is jsmith). Once created, email your username to LHR.

Research Your Commute Options

Visit UCLA Transportation to learn about your commute options including parking permit rates and public transit discounts.

If you are driving to campus, UCLA Transportation will provide you with two complimentary daily parking passes for your first two days on campus if your appointment is 43.75% or more. See Your First Day, Step 2 for more details.

Prepare Your Paperwork

All new hires and rehires are required to sign in with LHR. Be prepared to present required documentation for the I-9 Form along with your social security card, which is required by UCLA Payroll, at the time of sign-in. LHR staff will review your paperwork and documentation and answer any questions you have at your sign-in appointment.

Review and complete the following paperwork beforehand and bring it to your sign-in appointment. For all paperwork, please remember to: 

  • Reflect your name as it appears on your social security card
  • Use a permanent address (P.O. Box is not acceptable)
  • Do not print double-sided or staple paperwork
  • Forms cannot be emailed to LHR
  • Electronic signatures are NOT allowed on paperwork

Form I-9: Employment Eligibility Verification / List of Acceptable Documents  

You will receive an email from Tracker I9 to complete section 1 of the I-9 Form (after your new hire paperwork has been reviewed) to verify your identity and authorization to work and an email with a link to DocuSign to upload your documents.  Please review the list of acceptable documents to satisfy the I-9. During your sign in appointment you will be required to present your documents to the LHR Representative.

UPAY 585: State Oath of Allegiance, Patent Policy and Patent Acknowledgement 

Form must be signed and dated at the sign-in appointment in front of a LHR representative for countersigning.

Form U5605: Voluntary Self-Identification of Race, Ethnicity and Veteran Status

Form CC-305: Voluntary Self-Identification of Disability 

 
     
     

5.

Form UCRS 419: Statement Concerning Your Employment in a University Position Not Covered by Social Security  (if applicable) 

This form should only be completed by Limited Appointment New Hires.

Workers' Compensation - Time of Hire Pamphlet 

 
     
     

7.

Background Check Pre-Employment Waiver 

New hires are required to complete this waiver and undergo a criminal conviction background check through UCLA Police Department (UCPD). See Your First Day, Step 7 for more information.

 
     
     

8.

Initial Glacier Information Form  (if applicable)

Employees required to complete this form:

  • Employees with an Employment Authorization Card (EAD card)
  • Foreign Visitors on J1, H1B or TN visas
  • Pending U.S. Permanent Residents
  • Refugee & Asylee Status
If you are not a U.S. citizen, please be prepared to present additional documentation based on your citizenship status. 
  • Lawful permanent resident - Permanent Resident Card
  • Alien authorized to work:
    • Employment Authorization Card (EAD card)
    • Foreign Visitor on J1, H1B, TN visas - Passport, I94, DS2019, etc.

Get Ready for Your First Day

Make sure to bring the following to your sign-in appointment on your first day.

  • Completed new hire paperwork (see Step 8 above)
  • Acceptable documents for identity and employment verification (see Step 8 above, Form 1 and Form 8)
  • Social security card
  • If you're driving, cash to buy a daily parking permit (see Your First Day, Step 2)
